• A copy of the identity card of the applicant and spouse.
• A copy of Marriage Certificate / Marriage Registration / Divorce Certificate / his/her spouse’s Death Certificate.
• Single status confirmation letter from employer or Commissioner of Oath if self-employed for applicants who are unmarried.
• Latest pay slip or income confirmation from Commissioner of Oath if the applicant and his/her spouse are self-employed.
• EPF statement of the applicant and his/her spouse (if relevant).
• The report and record of the applicant and spouse with Central Credit Reference Information System (CCRIS) or CTOS which states the CCRIS report and record.
• License of the company / business registered with SSM / business experience supporting documents (if any).
